Does anyone manufacture a 49cc 4 cycle engine that has a counter-weighted crankshaft and no external flywheel?

I don't know of any small utility type 4-stroke engines of the configuration you describe. The reason few (if any) exist is because most small gas engines have a flywheel magneto ignition system.

Small Honda motorcycle engines have a fully counterbalanced crankshaft, for example, but they still have a flywheel for the magneto.
