When you run your chain with more than a 1/2" deflection this can happen. My OEM chain tensioner was garbage and let the chain become too loose.

That is one of the main reasons why I will always custom cut and fit my chains in order to to not even have to ever use a chain tensioner...I would much rather use the master link in removing the chain when necessary than to have a good chunk of my casing breaking off my motor from the chain binding up like that one did.
 
I fixed it up with Uniweld stick. Don't know if it will hold or not, but seems solid right now.
