Clutch Grinding sound after engaged clutch

While riding I hear a grinding sound when engaging the clutch. I always disengage the clutch when slowing down and reengage the clutch when accelerating again. Upon reengaging the clutch I hear a grinding sound. I just put a spring tensioner on, a new carburetor and a new spark plug the ngk b7hs 5110. I'm not sure if it's the spring tensioner. But it seems like it's running so much better with the new carburetor, spark plug and the spring tensioner. I dont have the problem of the chain falling off now.

Not sure what the grinding sound might be, but in the 1st and 3rd pic, is the case bolt backed out or is there a spacer making it stick out like that (the one towards the front on the left side)?
I would also trim the clutch cable so it doesn't drag into the chain, looks like it has a couple times.

Honestly it's an entirely different bolt I put on there because the original kept falling off. I have not had that problem with this bolt but I think its backed out a bit but that's as tight as I could get it . And I will trim the clutch cable tonight. Thank you.
 
For the grinding sound, pull the right hand cover off and see what falls out, I've had some bits of something or other in there before and the bike ran much better when I cleaned it out.
If that bolt doesn't go in any further, then put a couple washers there and some loctite to keep it in.
