Flower nut spun out stripped both nut and shaft --Why ?

No it was still in the right place

I had a similar issue only had about 10 miles on it. I heard a metal noise I thought. So pull the clutch cover and everything good . I squeezed the clutch lever looked good put it back together. I started it again still noise . I thought I’ll pull cover again wow. I happen to look at cover real bad groves really deep missed it first time . So I check it out really good found the nut on the pad wheel just barely on. I put lock tite on it and tighten really good . No problem since. I got lucky. Don’t know why it was lose I idled it a lot maybe vibrations or not tighten enough from factory.
