Wheels Messed up bearings

How come my bering on the forks and the coster brake keep getting fouled up. The bearings on the frount were kind of loose then I tightened them up and then they messed up. The other picture in the coffee can lid is of the berings on the sprocket side the inner ones. the the sprocket functions on. I think I have them adjusted right what is the best way to adjust them. The one on the coaster brake just gets crunch back there and the bearings pop out the one on the forks grind down to almost nothing. These are from late modle chines parts.

how hard do ride ?
if it happens often something is wrong
either wrong size berings or the race is chewed up or something

Sure looks dry to me. Repack with tons of grease.

Ya, they were kind of dry. Only about 50 miles on it. I put more greese on them. I will put even more this after noon.
