Hey, Amigo, just use that marvelous form the way it is written! It is one of the few things in this state that actually operate corrctly, even after surviving being processed thru a beurocracy (see I can't enen spell that convoluted word!)
The address is on the form, just read it, do hat it wants, then (if need be) look in the fee rate page and find the money needed (last year was 18.00).
Write your check, and mail it in, then wait for the magic day when yer lil plate comes in the mail!
